This bank information will be used by Precision Tax Advisors to submit the authorized Florida Department of Revenue payment for this filing. The $100 PTA fee is separate.
The final Florida DOR amount will be determined during PTA's review. This questionnaire does not itself file the return or submit the DOR payment.
